DIGITAL SCREENING MAMMOGRAM  

Appointment must be scheduled on or before December 31, 2012 and services must be rendered on or before January 31, 2013. Fee payable in full at time of appointment.

Coupon covers one screening mammogram and corresponding radiology interpretation.  Exam results requiring additional imaging or services are not included and are subject to separate billing and pricing.

Physician order must accompany coupon or be submitted to Reed Imaging Services prior to appointment
